I acknowledge and accept the responsibilities below: Confidentiality of student and school information. P's & Q's responsibility to provide trained and qualified staff. School's responsibility to provide a safe working environment for staff.

I confirm that the staff provided will adhere to the stated dress code.

I agree to report any incidents or emergencies immediately to P's & Q's Staffing.
